1. Items of Personal Information Collected
Our clinic collects only the minimum personal information necessary for medical service use.
Essential information for medical treatment is collected through patient registration forms without separate consent as required by the Medical Service Act. For additional services beyond treatment, a separate consent form for collection and use must be completed. There is no disadvantage in treatment even if the consent form is not completed.

2. Purpose of Collection and Use of Personal Information
Our clinic uses collected personal information for the following purposes.
All information provided by users will not be used for purposes other than those described below, and prior consent will be obtained if the purpose of use changes.
- Medical information: Medical services for diagnosis and treatment, and administrative services such as billing, payment, and refunds
- Additional service information: Medical information, academic information, and hospital information guidance
3. Retention Period, Destruction Procedures and Methods
Our clinic retains personal information for the retention period specified by the Medical Service Act and destroys it without delay afterward.
- Retention period: Medical records — 10 years
Additional service information is destroyed together with patient information or upon request by the data subject.
- Destruction procedure: Immediate destruction by the destruction method after the statutory retention period
※ Personal information for which 10 years have elapsed since the last medical record entry shall be destroyed immediately.
- Destruction method: Personal information stored in electronic file format is deleted using technical methods that prevent reproduction; personal information printed on paper is shredded or incinerated.

4. Rights of Users and Legal Representatives and How to Exercise Them
When collecting personal information of children under 14 years of age (hereinafter referred to as "children"), consent from a legal representative is always obtained.
To obtain consent from a legal representative, our clinic collects minimum information such as the name and contact details of the legal representative from the child, and obtains the legal representative's consent according to the methods prescribed in this privacy policy.
Users and legal representatives may exercise their rights by contacting our clinic via the internet, phone, or mail regarding personal information, and our clinic will take necessary measures without delay.
※ Personal information that is required to be retained by law cannot be modified or deleted within the retention period even upon request.
5. Withdrawal of Consent / Account Deletion
You may withdraw your consent for the collection, use, and provision of personal information at any time.
To delete your account, please submit a withdrawal application form or contact the Personal Information Protection Officer by mail, phone, or fax, and we will take necessary measures such as destroying your personal information without delay.
6. Third-Party Provision of Personal Information
Our clinic submits medical records to the Health Insurance Review and Assessment Service for medical fee claims pursuant to the National Health Insurance Act.

8. Measures to Ensure Safety of Personal Information
Our clinic has implemented various security measures as technical safeguards for the protection of users' personal information.
All information provided by users is securely protected and managed by security equipment such as firewalls. In addition, our clinic has established procedures for accessing and managing users' personal information as administrative measures, minimizes the number of personnel who process personal information, and conducts ongoing security training.
We also designate users of systems that process personal information, assign user passwords, and regularly update them.
9. Obligation to Notify Policy Changes
This privacy policy was established on July 20, 2020. In the event of additions, deletions, or modifications due to changes in laws, policies, or security technologies, the changes will be announced on our bulletin board at least 7 days before the revised privacy policy takes effect.
